How to read your Explanation of Benefits (EOB)
It's not a bill, but it tells you what you should owe.
After a claim, your insurer sends an Explanation of Benefits. It shows what the provider charged, what the plan allowed and paid, and what you may owe. Compare it with the provider's bill before paying.
The main lines
- Amount billed: what the provider charged
- Allowed amount: the price your plan agreed to
- Plan paid: what the insurer covered
- Your responsibility: deductible, copay, coinsurance or non-covered amounts
- Notes or codes explaining any denial
The rule
Don't pay a provider bill until it matches what the EOB says you owe.
